Who is responsible?

Which data?
Data to identify you (name, date of birth, citizen service number, address, phone, email), your insurance details and your medical record.
Why?
To provide your care, to work with other care providers when your treatment requires it, and to bill the care. This is laid down in the Dutch Medical Treatment Contracts Act (WGBO).
For how long?
We keep your medical record as long as the law requires: in principle 20 years after the last treatment. A registration form that has not yet been processed is deleted no later than 30 days after it has been handled.
Your rights
You may view, correct or have your data deleted, as far as the law allows. Email or call us. If you disagree with how we handle your data, you can file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
